Not sure, but that shape appears to be due to WLS. I'm betting that she used to weigh well into the upper 500s and that, if her face were visible, she'd likely be recognizable to this scene.

She had the wls because her partner at the time didn’t like her large & blamed all there troubles on it.

They divorced & we started dating & she’s been happy & gaining ever since
